A COLLABORATIVE APPROACH
What is Collaborative Care?
Collaborative care is based on creating a knowledge-based centre where ideas, research and treatment is shared in a multidisciplinary setting to provide exceptional care for individual cases that foster interdisciplinary collaboration. At Kootenay Integrative Health we meet to discuss best practices, health care options and how to best serve you.
Why is it Important to you?
As a patient you will experience the exceptional knowledge, skills and qualities of multiple practitioners. We meet frequently to discuss best practices and encourage a communication- focused collaborative health care environment. We want you to experience the best possible care.
Our mission is provide you with a progressive patient-centred experience in a setting where you are allowed to optimally heal and grow. You will experience health and care in a way that exceeds your expectations.
We provide exceptional collaborative healthcare in the form and manner that best serves you. We will consider the full spectrum of your biopsychosocial needs. Always.
We will be the first choice for exceptional full spectrum healthcare in the Kootenays. Our success in working with our patients will influence the health and strength of our community – making a difference in our patient’s lives and the lives of others.
